The successful applicant must Bachelor’s degree in either Social Work, Health & Human Services, Community Services or related field. At least one year working in customer service. Care Coordination Certificate within six months of hire. Must have Basic Life Support for Healthcare providers within three months of employment. Must possess a valid driver’s license.Salary is $25.17 – $28.94 per hour, plus benefits, may be employed under a contract.
