Join our Homecare/Hospice Department as a Respiratory Care Practitioner. This full-time benefited position works Monday through Friday with a rotation of being on-call every 4^thweekend. This 80 hours a pay period position plans and provides comprehensive assessment and care to patients needing home medical equipment and respiratory care.

Essential Duties:

• Obtains admission information and appropriate medical orders in accordance with the home medical equipment process.
• Conducts the initial assessment and determines eligibility for admission to the home medical equipment program.
• Develops an individual plan of care that respects the patient, family and/or significant others wishes and needs.

Educational Qualifications:

• Associates or Bachelors degree in Respiratory
• Licensed Respiratory Care Practitioner in the state of Wisconsin.
• CRT or RRT license.
• BLS upon hire.
• Experience with Respiratory DME Homecare preferred.
• Must have a valid WI driver’s license, proof of insurability, and reliable transportation.
